How the Progam Works

You fully complete and sign an application. (Print one here.)
Make a copy of the card or tax form or letter that shows proof of your financial need.
Write a check or money order payable to Animal Welfare Society for your portion of the spay/neuter fee as follows:
- Male Cat.............$35
- Female Cat..........$45
- Male Dog.............$55
- Female Dog..........$65
Mail the application, proof and your payment to:

- Lapeer, MI 48446
When you are approved, we call you with the name of a participating vet in your area.
You make an appointment with the vet and take your pet to the vet for the surgery. You pick up your pet after the surgery.
The vet will tell you how to prepare your pet for the surgery and what care your pet will need after the surgery.
The vet bills AWSMM for the surgery, and we pay the vet bill for the spay/neuter surgery. If you request additional services from the vet, you will have to pay for them yourself.

Eligibility

In order to qualify, you must meet at least one of the following criteria of financial need. You MUST provide proof of eligibility with your application. All personal information will be kept in the strictest confidence.

Medicaid card (adult, not child)
WIC (Women, Infants & Children)
EBT card (food stamps)
SSI (adult, not child)
IRS form 1040 showing income levels less than the limits below:
- 1-person household - $17,960
- 2-person household - $24,240
- 3-person household - $27,468
- 4-person household - $33,120
- 5 or more person household - $34,464

Fees (Pet Owner Copays)

The pet owner must pay one of the following amounts depending on the type and gender of the pet:

Male Cat.............$35
Female Cat..........$45
Male Dog.............$55
Female Dog..........$65

Having the pet owner pay a portion of the surgery charge allows us spread our resources farther to reach more pets. If the copay represents a true hardship for the pet owner, we will consider reducing the fee based on individual circumstances.
